Book and pay for your intake visit.
Download and complete the questionnaire, medical history, and medication forms.
We look at:
current symptoms and concerns
medical history
family history
medication use and exposure
lifestyle, stressors, and destressing
physical activity
your living arrangements
typical schedule
sleep schedule
relationship with food, and the types of things you eat, drink, clean with, and put on your body.
Sign in to the patient portal, sign practice paperwork, then upload forms and any labs you have
The questionnaire and interview help identify where to intervene first:
nutrient or calorie imbalances that prevent the cells from performing basic functions
cellular stress from pathogens, oxidative stress, or environmental exposures
sleep, hormones, and environment are in a continuous feedback loop with stress, happiness, gratitude, and healthy relationships
Amber will tie your symptoms together in a general manner taking into consideration possible nutritional deficiencies and lifestyle factors before we have our intake session. You will do most of the talking during the intake session. This is when the timing of events and exposures come to light. As you remember things and share your story you may correlate your symptoms with events in your life.
If your bloodwork is over one year old or predates your symptoms, you will need labs drawn.
We will schedule a follow-up session to fully explain what your tests reveal and discuss ways you can impact your health, whether it’s through lifestyle, diet, supplements, or self-care practices. Solutions and recommendations will fit your lifestyle, budget, and priorities. They will be driven by you and supported by research and current practices.
You will find your lab results and written recommendations, including long-term and short-term goals, and links to reorder supplements in your patient portal.
Get the support you need
We will schedule additional follow-up session(s) to track progress with short-term goals, and touch on the next steps and recommendations for additional lab work. The time between follow-up sessions and testing will depend on the severity of symptoms and the complexity of the plan.
